NetWare 6 inplace upgrade failed

During and Inplace upgrade, DIBMIG.NLM will not load because of public symbol errors.

Modules in the C:\NWUPADTE from a previous install would cause the Inplace Upgrade to fail.

Error While Contacting Security Domain Infrastructure Key Server: "server name".  Error #215

NetWare 6 installation fails, CCS does not initialize, abends in DS or DIBMIG

ERROR: "volume could not be mounted some or all volume segments cannot be located"

Hardware detect during install/upgrade does not detect MDAC.HAM driver.

Error extending schema during NetWare 6 install.

Error: The NetWare Cryptography Key File can not be opened or copied

How do I upgrade a NetWare 4.10 server to NetWare 6.0?
