Error: Can't create the boot partition

Error: Error writing to the partition table.

Error:  mm_create hotfix failed with code XXX

fix

This is a hardware issue in the disk channel.  Make sure the HBA is seated properly.  Run diagnostics on the disk. Update BIOS on the controller.  Check the termination of the drives.
