symptom

Error: Ds could not open local database it is inconsistent

Error: NDT files could not be renamed to NDS

Unable to create any files in the sys:_netware directory

DSREPAIR works for a little while after rebooting the server, then the errors come back.

fix

Server was mirrored, and the mirror was using up all the servers free blocks, even though he head plenty of disk space.  Breaking the mirror fixed the problem. 
To check the amount of free blocks
1. Load monitor
2. Select volumes
3. Press Tab on the volume in question
4. check Total Blocks and Free blocks.  Free blocks should be at least 10% of Total blocks.
