Error from dstrace log:
LDAPSub.performAddOperation() No destination DN because a placement rule did not match

cause

1. There were spaces in between some of the elements of the DN in the placement rule:
dn: cn=testuser,ou=myou, ou=myotherou, o=myo

2.  An ou is specified with the cn notation instead of ou.  Example incorect cn=myou,ou=myotherou,o=myo,dc=mydoc,dc=com.  Correct example ou=myou,ou=myotherou,o=myo,dc=mydc,dc=com.  If you are using the default users container, it has to be specified as cn=users,o=myo,dc=mydc,dc=com.

fix

1.  Remove spaces in between the elements of the DN in the placement rule

2.  Change the cn to ou.
